Format: Largely didactic but with much audience interaction

Health and illness are still understood and treated primarily as physical problems. This leads to linear processing of patients, specialisation, and fragmentation of care. Systems that successfully reduce overall healthcare costs and improve population health redefine the core product of healthcare as an effective, personal, long term, trusting, accountable personal relationship – experts, patients, and families working in partnership over time. Integrated care teams in long term respectful partnership redefine primary care and then result in entire system rethinking of roles, responsibilities, and functions.

After this session you will be able to:

  • Understand how dramatic improvements in healthcare are most likely to be found by dramatically changing outpatient services, particularly primary care.
