En5: Chronic conditions between hospital and primary care in Regione Lombardia’s reform
The recent redesign of health care system in Regione Lombardia, launched in January 2018, is centred on planned care for chronic patients resulting from a multidimensional evaluation of health and social needs of the patients, and based on inter-professional teams, including general practitioner, consultant, pharmacist, nurse, social worker.
Different levels of health needs and complexity of care were identified, covering a population of about 3, 350 000. Main results in delivering care and outcome are discussed.
After this session, participants will be able to:
1. to identify models for chronic disease management, aiming at effectiveness, efficiency and safety
2. to evaluate the sustainability of different options in healthcare redesign
3. to verify the reproducibility of the method present in a specific context
