Our services range from tertiary and national hospital specialist services, primary care, mental health, public health, children\u2019s services and many more. \u00a0Since 2016 we have partnered with four local councils through our Integrated Health and Social Care Joint Boards. Lothian started a highly ambitious Quality Management System programme in 2016 and formally agreed to a major acceleration over the coming five years, described within our Quality Strategy. This visit will demonstrate how person-centred, anticipatory care planning approaches are reducing Emergency Department attendances and acute bed days, including working with GP practices and care homes.<\/span><br><br><span style=\"font-weight: 400;\">This visit will demonstrate innovative approaches to anticipatory patient care across a hospital, primary and community-based settings in Edinburgh and the surrounding area. \u00a0All the visits will be to sites formally recognised for the quality of care they provide through external awards and validation.
