This session explores the Age-Friendly Health Systems movement, a growing global effort to deliver care that aligns with an evidence-based framework known as the 4Ms: What Matters, Medication, Mentation, and Mobility. Originating in the United States, the movement has improved care for over 5.4 million people and is now inspiring adaptation in diverse contexts worldwide. Join movement leaders and improvers from Ireland and the US as they share strategies for implementing the 4Ms across settings including hospitals, long term care facilities, outpatient services and more. We will spotlight Mowlam Healthcare\u2019s work in Ireland to embed Age-Friendly care within 20 long-term care facilities.
