Patients (micro) individual needs and goals should not only be leading in treatment decisions but also in the quality measurements on the meso and macro level (Society Personalised Healthcare).<\/p><br><p>During our workshop we will demonstrate how to support and execute the implementation of personalized care on the micro, meso and macro level based on well-studied real world cases that implemented personalised care successfully for people with rheumatoid arthritis and elderly cancer patients.<\/p><br><p>We show how to conduct a genuine conversation with patients based on metrics and context, to find out what matters to them and find the treatment that suits best.
