To meet the challenges of population ageing, a reorientation of health systems is necessary to provide integrated care for increasing the proportion of people with chronic diseases and disabilities. This is especially daunting for many Asian countries and jurisdictions in which many health systems are fragmented by pluralistic financing and provision. Instruments for integration are in development based on the five mechanisms of integration: professional, clinical, organisational, normative, and functional.
