Part 1: Scaling and Spreading Improvement: A healthcare Community of Practice for Shared Learning Within and Across Organisations
Discover how the success of The Singapore Healthcare Improvement Network, SHINe, a collaborative platform for cross-organization’s learning in Singapore’s healthcare system which aimed to reduce harm in patients by 30% in 3 years through Medication Safety, Surgical Safety promotion and Healthcare Infection Prevention workstreams, laid the foundation for TeamSCALE™, an ambitious next step in scaling and spreading best practices within and across SingHealth institutions and Care partners. Building on these achievements, TeamSCALE™ is designed as a Community of Practice (CoP) to embed SHINe’s principles within organizations. This session will explore the evolution from SHINe to TeamSCALE™, focusing on strategies for fostering collaborative learning, scaling improvement efforts, and sustaining a network of champions. Part 2: Health for life–Improving the ‘Big Dots’ in Singapore with Redesigned Large Scale Improvement Initiatives
Singapore’s health strategy uses a population-based approach to encourage Singaporeans to live healthier lives. This session describes how the National Improvement Unit (Singapore) utilised a modified Breakthrough Series collaborative to improve Diabetes Care and modified action lab design to improve other chronic disease care management in primary care. This session describes how the two phases of the National Diabetes Collaborative utilised a modified IHI Breakthrough Series Collaborative methodology to reduce diabetes admission (Phase 1) by 10% (in 18 months) and decrease average HbA1c of diabetic population by 1% (in 12 months) with over 30 teams across tertiary and primary care settings participating in this collaborative. This project shares lessons learned from this national collaborative, with varying results across sites, the spread and scaling of interventions nationally.

Part 3: Enhancing Efficiency and Quality in Healthcare: Value Management at the Point of Care
This session focuses on an innovative approach to quality improvement, emphasizing better outcomes and patient satisfaction while maintaining or reducing costs.
