In this session, we will elaborate on our tailored patient safety framework for the dialysis unit. This safety and quality model empowers us to effectively address errors, establish key performance indicators, design robust quality performance metrics, and safely develop pioneering programs. We will share on how we elevate our commitment to patient safety and transform the standards of care. Part 2: Sustainability and Effectiveness of the Introduction of Tunnelled Haemodialysis Catheter Management in the Community – the Singapore experience


This session, presented by Singapore’s National Improvement Unit (NIU), highlights how integrating improvement and implementation science enables the scale and spread of large-scale healthcare initiatives. Focusing on a national project addressing Tunnelled Haemodialysis Catheter (THC) complications, we showcase how this hybrid approach reduced hospital admissions, saved SGD 560,000, and enhanced patient outcomes through community-based care.Participants will learn the frameworks and strategies used to scale sustainable change, with a focus on aligning iterative testing (PDSA cycles) with implementation science principles such as acceptability, adoption, and sustainability.This session will provide actionable insights into driving systemic, patient-centred healthcare improvements. It will be especially valuable for leaders and change-makers looking to navigate challenges of scaling innovation while maintaining safety, quality, and cost-effectiveness.

Part 3: Safe Communities, Healthy Lives: Advancing Population Health Together


The session will explore the critical role of community nursing in promoting population health and enhancing community safety across various domains. It will focus on the innovative strategies and best practices on key areas such as medication safety, home safety, infection control practice, falls prevention, and safe management of chronic disease. The discussion will also highlight the importance of caregiver training, telehealth and remote monitoring in enhancing care delivery and accessibility. Participants will gain insights into successful collaboration between healthcare providers, community organizations and local stakeholders to improve healthcare access, reduce acute care utilizations and enhance overall well-being.
