F4: The “F” Word!! – Learning from failure

When we train individuals in quality improvement we often describe the need to learn from failure. But how often do we truly do it in practice? This session will present an honest account of setting up QI programmes in large mental health and acute settings in England and Wales. The successes and failures, the rollercoaster journey, the learning, and the resources you need to survive and grow.

After this session, participants will be able to:

1. Identify failure early on and learn what to do next
2. Use the key tenets described as a framework to progress their QI programme regardless of current context
3 Work creatively with the messiness of change, and manage the psychology of failure in QI
