This session will explore an improved method for incident investigation – Root Cause Analyses and Actions (RCA2). RCA2 introduces human factors thinking and practical tools to drive development and implementation of sustainable, measurable actions that prevent future harm to both patients and the workforce. Participants in this session will work with experts to understand and practice a framework to transform how they think about improving the safety in their organizations.
