PART ONE: Guiding what matters: personalised approach, metrics and conversations

The transformation to person-centred care is broadly embraced however real world examples are scarce. Patients (micro) individual needs and goals should not only be leading in treatment decisions but also in the quality measurements on the meso and macro level (Society Personalised Healthcare).

During our workshop we will demonstrate how to support and execute the implementation of personalized care on the micro, meso and macro level based on well-studied real world cases that implemented personalised care successfully for people with rheumatoid arthritis and elderly cancer patients.

We show how to conduct a genuine conversation with patients based on metrics and context, to find out what matters to them and find the treatment that suits best. Research shows the applied treatment is often different from common practice and better results are achieved.

And we’ll have a well-informed dialogue with the audience about routes and conditions to spread this person-centred approach.

After this session, participants will be able to:

  • Include relevant metrics and context in treatment selection with patients

  • Structure and appreciate a genuine dialogue with patients about treatment goals and best suiting treatment options

  • Get to grips with factors supporting development and spread of a person-centred approach
