Medication errors represent a significant challenge for healthcare systems worldwide due to their impact on patient safety, health outcomes, and healthcare costs. These errors can occur at any stage of the medication process, including prescribing, dispensing, administering, and monitoring, leading to a variety of adverse effects and threats to patient safety.

In this interactive workshop, you’ll learn more about the common causes of medication error, and how QI methodology can be applied simply and powerfully to reduce risk and improve safety and reliability. While the focus will be on medication error, the principles discussed will also be applicable to avoiding failure and adverse events in wider settings.
