Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA), designed to create systems of health and social care that anticipate and respond to the unique needs of older adults. This session will present a framework that has been proven to improve the care and experiences of older adults and generate conversations around what age-friendly health systems should look like globally.
After this session, participants will be able to:
- Assess, document, and act on the 4Ms of evidence-based, age-friendly care.
- Describe ways different types of health and social care organisations provide age-friendly care to adults globally
- Apply strategies and tools to spread the 4Ms, including engaging leadership to support implementation of age-friendly care.
- Co-design future models for sharing learning and resources across global systems
