Internist in geriatric medicine, Saxenburgh; Netherlands

Carolien Wijsman (MD, PhD) is an internist – geriatrician working at Saxenburgh Medical Center, a general hospital based in a rural Dutch area. Besides working in a hospital setting she has gained experience in working as a specialist in elderly care in a community-based setting working in networks to strengthen care for frail elderly living at home.

With Saxenburgh Medical Center, she aimes to transform the care for elderly towards a network-oriented collaboration with health-care workers involved in the care for frail elderly, such as elderly care physicians, casemanagers dementia, social workers and general practitioners. The objectives are to enhance patient empowerment regarding health-related goals in life, and to deliver personalized care provided at the most appropriate location.
