PART ONE: New ideas for spreading interventions that work: an integrated model for spread and scale-up


The bloom of 1000 improvement flowers can quickly wither if not directed by sustainable and scalable designs. Come and hear more on IHI’s new integrated model of Spread and Scale, and recent field tests of this approach.


After this session, participants will be able to:



  • Understand an actionable integrated model for spread and scale

PART TWO: Scale or fail: using implementation science frameworks in the real world


This interactive session will provide advanced improvement practitioners, who are involved in spread initiatives, with insights into how implementation science frameworks and models can be applied practically to support the scale and spread of improvement in real-world contexts. Participants will be introduced to the Hexagon Discussion and Analysis Tool and will hear how the Health and Social Care Quality Improvement (HSCQI) team in Northern Ireland has integrated its 6 domains into a novel, evidence-based tool for assessing readiness. The HSCQI Scalability Tool, which was co-produced with lived experience representatives and colleagues from across the health and social care system, assesses readiness for spread at an individual project level and at a system level. Participants will have the opportunity to experience an interactive version of the tool. This session will be delivered in partnership with HSCQI’s lived experience colleagues, whose input into the co-design of the tool was key to its functionality and acceptability.


After this session, participants will be able to:



  • Understand the challenges of leading change and spreading improvement across an integrated health and social care system

  • Describe a range of implementation science frameworks and models and how they relate to improvement science

  • Describe the Hexagon Discussion and Analysis Tool and the HSCQI Scalability Tool and how they can be used practically to support the assessment of readiness for scale and spread at a local project level and system level

  • Learn lessons from a successful spread initiative that can be applied to their own spread projects
