Part 1: Scaling and Spreading Improvement: A healthcare Community of Practice for Shared Learning Within and Across Organisations 


Discover how the success of The Singapore Healthcare Improvement Network, SHINe, a collaborative platform for cross-organization’s learning in Singapore’s healthcare system which aimed to reduce harm in patients by 30% in 3 years through Medication Safety, Surgical Safety promotion and Healthcare Infection Prevention workstreams, laid the foundation for TeamSCALE™, an ambitious next step in scaling and spreading best practices within and across SingHealth institutions and Care partners. Building on these achievements, TeamSCALE™ is designed as a Community of Practice (CoP) to embed SHINe’s principles within organizations. This session will explore the evolution from SHINe to TeamSCALE™, focusing on strategies for fostering collaborative learning, scaling improvement efforts, and sustaining a network of champions. Part 2: Health for life–Improving the ‘Big Dots’ in Singapore with Redesigned Large Scale Improvement Initiatives


Singapore’s health strategy uses a population-based approach to encourage Singaporeans to live healthier lives. This session describes how the National Improvement Unit (Singapore) utilised a modified Breakthrough Series collaborative to improve Diabetes Care and modified action lab design to improve other chronic disease care management in primary care. This session describes how the two phases of the National Diabetes Collaborative utilised a modified IHI Breakthrough Series Collaborative methodology to reduce diabetes admission (Phase 1) by 10% (in 18 months) and decrease average HbA1c of diabetic population by 1% (in 12 months) with over 30 teams across tertiary and primary care settings participating in this collaborative. This project shares lessons learned from this national collaborative, with varying results across sites, the spread and scaling of interventions nationally.

Part 3: Enhancing Efficiency and Quality in Healthcare: Value Management at the Point of Care


This session focuses on an innovative approach to quality improvement, emphasizing better outcomes and patient satisfaction while maintaining or reducing costs. We aim to introduce the “Value Management” concept for healthcare quality improvement. Based on Lean accounting principles, helps organizations shift from volume-based to value-based systems, allowing them to thrive, not only survive. We will discuss various tools, review the improvement model, and provide examples from facilities that have implemented this model. An evaluation framework for improvement initiatives will be provided to help participants quantify the program’s impact. Additionally, we will share specific examples of cost reduction and quality improvement from various healthcare systems worldwide. Officially opened in July 2024, Woodlands Health Campus is the newest integrated healthcare development in Singapore. It features integrated care across pre-hospital, acute, ambulatory and community settings. W8: But why did it all make perfect sense at the time? Enhancing incident investigations using Human Factors (Part Two)

This workshop teaches participants to bring the elucidation of human factors to the fore when investigating clinical incidents, with the aim that impactful and sustainable improvements can be made to systems and human performance.


Participants will learn experientially by conducting interviews with interactive role players, portraying the first, second and third victims of incidents crafted from actual occurrences in our campus.


Those who conduct incident investigations or are in positions of accountability to make decisions on post-incident recommendations should attend this workshop.


Through the workshop, participants will better appreciate how the scientific rigour of investigation is applied in tandem with the art of understanding human behaviour and frailties of healthcare providers. They will explore the various types of human errors to enhance their ability to conduct nuanced investigations.


With this enhanced appreciation, we envisage that participants will be better able to identify latent error, implement interventions that address root cause of human and system error, and create safer and more reliable environments for patients and healthcare providers.


After this session, participants will be able to:



  • Apply human factors concepts on how to conduct system-based incident investigations that transcend beyond individual blame, incorporating models of human information processing and taxonomies of unsafe acts.

  • Practice crucial skills such as conducting RCA interviews and speaking with staff in a fair, learning-focused way after adverse events, utilising human factors principles to enhance communication and understanding.

  • Develop effective recommendations using a hierarchy of interventions model to create sustainable changes that address root causes, considering various types of errors including perceptual, decision, and execution errors.
