Population health improvement is being increasingly focused on by healthcare systems and their leaders yet it is notoriously complex. In this session we will share examples from the UK of how a country, health system and institutions have approached population health improvement in our different contexts.
We will share and compare our leadership lessons highlighting the human side of change, alongside approaches to strategy and implementation. We will demonstrate how we have engaged the patients we serve, our staff, and our communities in this journey.
We will share data and stories to highlight how improvement approaches have been at the heart of our work and demonstrate the impacts these are making in our populations.
We will use our lessons to help you identify ideas and develop a plan to take learning back to your own healthcare systems.
Objectives:
- Access learning from different approaches to population health improvement
- Identify key lessons from leadership, strategy & implementation approaches relevant to your context
- Develop ideas & plan to apply these lessons to improve population health in your own healthcare system
