Health systems are implementing a range of solutions to support avoidable hospital admissions but how do we know that we are keeping patients safe when we are treating acute health needs? There is much evidence related to patient safety in acute care settings and as a range of alternative approaches develop across services, it is important that we apply the same principles for patient safety. IHI’s Framework for Safe Reliable and Effective Care offers a structure to support this area of development.

This session will explore how improvement can support delivery of safe care to people who have avoided hospital admission and will share examples of how this is being developed within the health system.

After this session delegates will be able to:

  • Describe a range of alternative care services that prevent the need for acute hospital admission

  • Understand the importance of managing patient safety in alternative settings to hospital

  • Consider how improvement approaches can support local work in this field of patient care
